Remember that health forms are due TONIGHT for everyone attending summer camp.
*The only participants whose forms are completed totally are: Alex B, David H, Patrick J, Scott J & Tom M.
Everyone else has something that still needs to have the following completed:
Notarizing Part A for all scouts, signatures on forms, Part Cs, and insurance cards or immunization information (Current at last physical or as of 2014 is NOT sufficient. You must have DATES of immunization. Adults: The last Tetanus booster date is required).
OR
If all of your forms are expired (includes everyone who went to camp last year), you need to complete 2 copies of the new 2014 version of the medical forms: Parts A, B, & C + an insurance card(front & back).
Other scouts/adults: if you have not submitted the new 2014 version of the health forms since 3/1/14, your forms are expired and you need to get your forms completed and notarized in order to participate in any event.
